Obtaining a Library Card  

Residents and Non-Residents

For applicants 18 years and older:

A resident of Carroll County or Madison County or counties contiguous to either including southern Missouri may apply for a library card in person at any of the libraries. The resident completes a library card application in full and presents proof of current mailing address. This proof may be a current driver’s license, a utility bill, printed check, printed identification (medical card, company ID), tax receipt.

For applicants under 18 years old:

Students and children residing in Carroll County or Madison County or counties contiguous to either, including southern Missouri may apply for a library card. They are required to have either their parent’s or legal guardian’s signature on the application. Parents and legal guardians must present proof of current mailing address with their child’s application unless the parent or legal guardian is in good standing and has an up-to-date address on file.

A library card is issued the same day as application is received. Limits on the number of items checked out on the first day of library card use may be set in individual libraries.

This policy also applies to temporary residents who should provide a local mailing address as well as a permanent mailing address.  

Lost Cards

Patrons are asked to contact the library immediately if the library card is lost or stolen. The patron is responsible for materials checked out on his/her card until the loss has been reported. Replacement cost of a lost library card is $1.00. Patron must show identification when applying for a replacement library card.

Change of Address

Cardholders are asked to inform the issuing library of any changes (name, address, phone, email address) to the application in a timely manner.  Address checks will be performed once a year.
